Rehabilitation Services Redesign

The Walton Centre NHS Foundation Trust, St Helens and Knowsley Teaching Hospitals NHS Trust, and Royal Liverpool and Broadgreen University Hospitals NHS Trust have established a new service of specialist assessment and rehabilitation for those who have suffered traumatic injury or illness.
Patients are referred and treated at designated units offering different levels of care. The newly redesigned service at The Walton Centre provides early rehabilitation for patients with intensive rehabilitation needs and consists of 10 specialist hyper-acute rehabilitation and 20 specialist rehabilitation inpatient beds.
Newly redesigned St Helens Hospital and Broadgreen Hospital caters for less intensive rehabilitation needs with 35 inpatient beds and a therapy out-patient service across two units. Unit facilities include individual and group therapy, gym and vocational activities.
Resources were pooled to support the service redesign with a combined budget, management and teams. A single point of contact was introduced to increase access and ensure patients received specialist treatment at the earliest opportunity.
Patients benefit from input of professionals, and 96% of patients said they were satisfied – or very satisfied – with their rehabilitation.
The Walton Centre FT won Specialised Services Redesign Award at the HSJ Awards 2015.
