AF Pathway

This clinical pathway has been prepared to support providers, public health commissioners and NHS commissioners to develop services and improve clinical practice for people with, or at risk of, atrial fibrillation (AF).

The AF Pathway aims to provide a practical guide to encourage discussion and collaboration between stakeholders and help deliver clinically effective and coordinated holistic services for people with AF or at risk of AF.

With the focus on care, the AF Pathway is also designed to support stakeholders in reducing AF-related strokes. 

The short and long-term aims of implementing an AF Pathway are based on:

  • Prevention of atrial fibrillation
  • Early detection of atrial fibrillation
  • Treatment of atrial fibrillation in acute and long-term settings
  • Reduction of complications (e.g. stroke, thrombo-embolic disease and heart failure)
  • Support at the end of life

The pathway is currently being used across Cheshire and Merseyside with early indications demonstrating an increase in anti-coagulation.
